CHSP is designed for those who need minimal support to maintain their independence and may be short term. Packages are designed to be flexible within funding limitations.
CHSP's can provide basic assistance with household tasks and may, as an example, provide some assistance with:
- Domestic assistance
- Personal care
- Home maintenance
- Social support
The services that make up your package will reflect your requirements and preferences within the scope of the program. Services will be reviewed regularly or as needed and changed as needs change. This will always be done in consultation with you and any significant others. One-off and short-term changes can be organised with My Aged Care.
To receive a CHSP you will need to have a Regional Assessment Service (RAS) assessment before being approved for a package. Once applied, and when a CHSP becomes available, a Coordinator from Alwyndor will arrange a mutually convenient time to meet with you in your home to develop an individual service plan. Other significant people are welcome to attend at your invitation, including a family member, friend or advocate. A Service Agreement is completed that details the services to be provided by Alwyndor and when they are to begin.
Who is eligible?
Older people who:
- Need minimal support to maintain their independence
- Are approved by Regional Assessment Service (RAS)
- Over 65 years
